The one-time service charge entitles you to have your Radio History listed
on 440: Satisfaction (your radio history will be added within 10 days of our
receipt of this agreement and your payment).

1) Submit your/his/her radio history. Please use the on-line Radio People
   Submission Form (at http://www.440.com/namesub.html) or send the
   following info to autosub@440.com:

   For each station:
   Station call letters, year you started/left, position held, name used ...
   and, what you/he/she is/are doing now.

2) Print, sign and enclose this agreement with your payment of US$25
   (please make check payable to 440 International Inc.) and mail to:

440 International Inc. Radio History Listing Agreement:

I hereby certify that the facts I have submitted to 440 International for
publication on the 440: Satisfaction Web site(s) are true to the best of my
knowlege, and I assume full and complete responsibility for the information
submitted. I hereby agree to indemnify and hold 440 International Inc.
harmless from and against all demands or claims of liability because of
information I have submitted. Additionally, I understand and agree that it
is my sole responsibility to notify 440 International Inc. of any changes
and/or updates to the data I have submitted.

I further understand that every effort will be made by 440 International
Inc. to ensure the accuracy and completeness of information submitted by me
and published by 440 International Inc. If I do spot an error, I will bring
it to the attention of 440 International Inc. by sending an e-mail to:
